I bet that John Romita, Sr. did a rough sketch and then Johnny tightened it up a bit. Tom Palmer inked it, and Frank G D’Armata colored it.
Maybe vice versa on the penciling duties. JR was written first, so it might be his rough that his Dad edited. Dunno. Looks like both of them though.
John Romita, Sr. is getting up there in his years, so I would not be surprised if he is needing help with his work these days.
