In Dr. Mario's official artwork for Super Smash Bros. for Wii U/3DS, his tie and collar are done up properly (top) while during gameplay, they are loose (bottom).
It is unknown if this is an oversight or if it is supposed to be an intentional humorous detail suggesting Dr. Mario only adjusted his clothes to have his official photo taken but does not care about his appearance otherwise.

Left: packaging of officially licensed "Mario Pikachu" hanafuda cards from a 2016 Mario/Pokémon cross-promotion event, from Japan.
Middle: this is actually a reference to the packaging of officially licensed 2015 Mario hanafuda cards.
Right: this, in turn, is actually a reference to the packaging of one of the earliest decks of hanafuda cards produced by Nintendo at the end of the 19th century, featuring Napoleon Bonaparte.

Top: an unused debug menu found in the finished version of Super Mario 64 lists Tall, Tall Mountain as "36 DONKEY % SLID2". As the course contains Ukiki the monkey, for decades it has been assumed that "donkey" here was a misspelling of "monkey" rather than a deliberate reference to Donkey Kong.
Bottom: however, development files for the game that have surfaced in 2020 contain the original file names for the models of the areas, where the model for Tall, Tall Mountain is called "36DONKSLID.bin". This suggests that "DONKEY" in the internal name was not a misspelling (as misspelling it in two separate places the same way is highly unlikely) and that Ukiki the monkey was possibly supposed to be Donkey Kong, or somehow related to Donkey Kong, at some point during development.

Top: a pre-release screenshot of Mario Superstar Baseball, published on the IGN gaming news website in 2005 (note the IGN logo in the bottom right).
Bottom: in the game, the preview image for Mario Stadium uses that exact screenshot, including even part of the IGN logo, seen overlapping Mario's shoe.
This indicates that the developers, despite having full access to the game's code and being able to take any kind of preview screenshot they could want without any restrictions, instead went online and took a screenshot from a gaming journalism website without even editing out the logo.

In Super Mario Odyssey, it is possible to erase Mario from existence using a glitch.
Top: if Cappy captures a Hammer Bro in Assist Mode while Mario is holding a turnip so that the turnip immediately falls and kills the Hammer Bro as he is being captured, Mario will cease to exist. All objects in the scene will briefly stand still, as though frozen in time.
Bottom: when the Hammer Bro respawns, Cappy will automatically capture him, but since Mario is no longer present, the player does not have control over the Hammer Bro. Instead, the Hammer Bro simply performs his usual enemy behavior forever, without player input. Mario will not return and the game must be reset to continue.

Top: when the pre-release Japanese box art for Mario Party 9 was revealed in January 2012, Nintendo was criticized by various online outlets for leaving Luigi off the cover (note how all 11 other playable characters are present, but Luigi is not).
Bottom: in the 2 months before the final box art was revealed in March 2012, the artwork was changed slightly by moving around some elements and darkening the sky; in addition, Luigi was added due to popular demand, although he is by far the least visible character on the box (he can be seen in the tower of the castle in the top left, directly to the left of the logo).
