I think that they do instinctually, but don't realize it. I'm all about a protective Alpha, that's literally my cocaine.

Subconsciously they are making sure that their Omegas are safe. They growl at people getting too close or shielding them with their bodies without realizing it.

In some headcanons Alphas can smell it in the scent before the Omega realizes it, it doesn't work like that for me. At least not exactly. At first, an Omega's scent heightens and the Alpha becomes more obsessed with smelling them. The pregnancy smell doesn't kick in until a little bit later. The pregnancy smells also vary.

Either way, for some Omegas the Alpha's behavior makes them go get a test, Omega misses a heat, Omega just has a feeling, or the Alpha smells it later on.

I'm strictly going to be talking in terms of my AU. I swear if someone asks me "but what if this" or "what if that", I'mma mclose it and wish all your bacon burns. All this depends on your AU.

I don't know much about percentages of irl multiple pregnancies, but I would think that it's either doubled or tripled in Omegan pregnancy.

I have multiples be standard in Omega pregnancies. It is common for them to have up to four pups in a litter. There are even cases of bigger litters. Don't know why this happens, but's fun to me.

Complications for just having multiples do lessen, but that doesn't mean that there isn't a possibility for it. Even "perfect" irl pregnancies can suddenly have complications. It's just a roulette.

I think I have mentioned this before, but pups in the Omegaverse are generally easier. Especially in multiples. Of course, they whine, cry, scream, poop, pee, need food, and everything in between, but it is significantly less. If anything, parents know before hand what their pups need based off scent. Like if they hungry? They'll smell it before the baby freaks. The baby is lonely, they'll feel that in their familial bond. They are just way more in tune with one another.

I really don't know. It's your story. I would take the regular symptoms of Omega pregnancy, I'm sure I have headcanons about it, and make them subtle.

They nest, but randomly. They are clingy, but not really. It's really up to you.

The only thing I would suggest is that if they were in an abusive relationship, maybe their ex somehow damaged their scent glands? So no one smells the pregnancy hormones.

It depends. I mean you can make it however you want. Lol. Omegaverse biology is up to your interpretation. This is not really a clear-cut answer. Everyone is gonna give you a different answer, dawg. 🤷🏻‍♀️

I mean, yeah. Lol. Think of how cptsd and ptsd work. They could be scared of just about anything because of the trauma. Especially if they have certain triggers.

And knotting depends on the person's AU. In mine, an Alpha always knots, but there are varying degrees of how big said knot is. Whether it's huge during their rut or an Omega's heat or small because they're getting a blow job. It all just depends.

So really, you gotta sit and think about how you want the knotting to work. I've read some AUs where people have it where an Alpha never knots to only knotting with a bonded partner. It just depends on what works for you.

The main one would be a scent change, hands down. There would be subtle pheromone changes, enough to heighten their own instincts into feeling more protective.

If it's not that, they'd notice that their Omega isn't producing the common heat pheromones they usually get. Seeing as the Omega wouldn't be getting their heat any time soon.

Also the Omega would become clingy or nesty.
