This is a California Kingsnake! They are well known for thinking everything is food. They’re opportunists, so in the wild they eat just about everything. They’re well known for eating rattlesnakes, and are therefore “kings” of snakes. As a result they are resistant to rattlesnake venom and pound for pound the strongest constrictor (since it takes a lot of constricting to take down another snake).

They are also pretty docile to humans and have become a common pet! They’re sturdy and have not many complicated husbandry requirements, making great snake pets for beginners. Except for the occasional thinking your hand might be food situation. I have found that target training helps with this, and being so food motivated it’s pretty easy to train them at any age.

They can come in a variety of colors and patterns, but the most favored is the normal black and white banded California Kingsnake. And totally understandable, they’re so cool!!

Harm Reduction Ideas for Substance Use Disorder

Recently I have been listening to a podcast called The Curbsiders Addiction Medicine. If you are a clinician that works even sometimes with people who use substances (every clinician ever), it is a fantastic look at all the harm reduction practices you can use to make these individuals safer. Plus, you get free CME.

I’m hardly going to do the podcast itself justice with this post, but I wanted to share some things I learned from it:

If the dangers of using substances (social and legal consequences, time commitment, health problems, money problems, etc…) was a deterrent, people wouldn’t be doing it. But it’s not. Because uncontrolled substance use is a chronic disease that generally does not get better without treatment. When people are treated, not only do they generally use less, but they have a much lower chance of death and a much higher chance of a happy, productive life- whatever that means for the patient.

Previously (even a few years ago) we hung such treatment on the requirement that people be abstinent from substances in order to receive help. This works for some people, but far from everyone.

The evidence shows that best thing we can do for many individuals is to make their use safer and less of a burden on their life and health. This is called harm reduction, and it WORKS.

Here are some evidence-based ideas for how to help your patients:

  • Create a space where you are working together with your patient and following your patient’s lead. Do they want to become abstinent? Great! Do they want to use less or use in a more controlled way? Also great! Do they want to continue use in a safer way? You guessed it, also great! Support them in whatever their goal is
  • Provide or prescribe safe, clean tools of use. Things like clean needles, Pyrex pipes, and straws. This decreases rates of infection and abscesses
  • Prescribe medications that reduce cravings or reduce/eliminate withdrawal (methadone, buprenorphine, topiramate, bupropion, naltrexone) without requiring abstinence
  • Teach people safer use practices and safer routes, such as rectal (booty bumping) or oral (parachuting) instead of injection drug use
  • Prescribe PrEP if people are at risk of HIV without requiring abstinence
  • Test for and treat the consequences of substance use (such as HIV and Hep C) without requiring abstinence
  • Provide fentanyl and xylazine test strips so people know what is in the substances they are using and can adjust doses/use pattern accordingly
  • Recommend Never Use Alone hotlines to prevent overdose death or better yet, take turns using with a buddy
  • Prescribe naloxone to anyone who uses any substance- nearly all street drugs are contaminated with synthetic opioids and naloxone is an effective way to prevent deaths
  • People use substances for a reason, especially early in their journey- pain, coping with depression/other mental illness, ADHD, and social issues like being unhoused. Treat the problem if you can find it, and you can help people significantly decrease use or use in a more controlled way
  • Be aware that return to use (or return to uncontrolled use) is a thing you can plan for with the patient and manage before it even happens

It’s hard sometimes to change the idea of addiction/substance use disorder as something that can only be treated as a reward for staying sober. But thats why so few people seek treatment for it. The evidence does not equivocate. Harm reduction WORKS.

Salt-rusted unprotected steel panels... Meltwater getting into poorly constructed and poorly isolated electronics... Stuck in snowdrifts that a real truck would have been able to deal with... Oh, those are indeed happy images. Yes indeed...

There should be Cybertruck Winter, like Fat Bear Week. Where we see which cybertrucks fall first and which ones make it to the end in usable condition.

Dispatch from the far northern hemisphere and have witnessed a Cybertruck in the winter wilds.

We're early enough into the snow season in that the damage isn't obvious. My guess is that exposure to road salts are really going to destroy these ambulatory dumpsters, but we won't start to see that until spring. Road salt is difficult to impossible to get off in a regular car wash, and we know that Cybertruck can't handle even that.

On the one I saw, any metallic shine that the Cybertruck had was completely lost in a combination of cold winter temps, light street grunge, and lower ambient sunlight. It was the same color as my friend's early 2000s silver pickup truck. One of the big draws, imo, is that stainless steel panelling and to see it turn into the same shade of grey as one of the most popular truck colors twenty years ago would be disappointing to me. It's not special anymore.

Local Cybertruck enthusiasts who are salty dogs at winter driving have started vinyl wrapping their automotive basket cases. The trend seems to be to go from the door windows down, which gives them a beach cooler vibe that is similarly underwhelming.

They’re already having issues! The head lights are sunken in for some reason. This means there is a shelf to hold snow in front of the lights and block them.

Now, every car has to have the snow cleared off the headlights before you drive, but this is way worse. That shelf collects snow as you drive. People have to pull over and clear the snow off mid-drive because they lose their headlights.

WHY ARE THEY BUILT THAT WAY

satoru, you think, is annoying.

he stabs one of your hairpins into place, the metal chiming, a sweet summer sound. sunlight catches in the ornate petals of it. you reach up and adjust it; it's cool against your fingertips.

"satoru."

he doesn't look at you, picking up another hairpin, this one burnished with age, its pearls gleaming, moon-touched. you wonder when he stole it from his grandmother.

he's more careful with this one, tucking it into your hair with long pianist's fingers. you tilt your head to help him, but he nudges you back into place.

you glance at him in the mirror. he's wearing sunglasses today; you can just make out a sliver of blue, the sky come down to earth. heaven devoured.

he reaches out for another pin.

you grab his wrist. infinity is slick against your skin, a glass wall, but then you feel the warmth of him. the quiet rhythm of his pulse.

"i don't like you this quiet," you say.

"i don't like you getting married," he returns.

you hum. "is that what you're sulking about?"

he makes an outraged noise. "i'm not sulking."

"you are."

"am not!"

you eye him in the mirror, raising a brow.

he huffs. slips another hairpin into place. he runs a gentle finger over it; it clinks quietly. you think of the windchimes that hung in the plum tree, the way your mother closed her eyes to listen to their song.

she was not invited today.

you take the final hairpin before satoru can. it slides into place perfectly. your brother's hands slips to your shoulders; he squeezes, one long, hard pulse.

"he won't treat you right," he says.

you tilt your head, considering. naoya is oddly careful with you, despite his sharp pride and even sharper tongue. you think of the way he looks at satoru and sigh.

"i think he will," you say.

"because?"

"because he thinks of me as something of yours."

satoru's grip tightens.

"he—"

"—is a zen'in," you say. "really, it's to be expected."

satoru pauses. then, slowly, he reaches up and takes off his sunglasses. you meet his eyes, the burning comet-tail of them.

"i'll kill him if he hurts you."

you smile.

"not if i kill him first."
