Twice in the last several episodes, they have made a point of showing us Cas getting shot in the chest and just shaking it off.

Twice in the last several episodes, they have made a point of drawing our attention to his weakening grade.

This feels very ominous. I don’t think Cas is going to get killed off, but one of these times, he is going to take a bullet and not be able to recover like before. One of these times, he will be so low on grace that being shot is as dangerous to him as a human. Cas is becoming more and more human and I’m here for this transition. I’ve been waiting since season 8 when every damn angel that talked to Cas pointed out that he would eventually have to choose: angel or human. He’s toed that line a long time. But at some point I feel like that is going to come to a head and when it does, will choosing to be human get him out of his Empty deal?  

Ugh This character’s story is so good! What a beautiful arc we have seen Cas go through.

I’ve wondered if it’s metaphorical, as well as (potentially) foreshadowing, because Cas is actually shot in the heart region (left side of the chest) both times, if you look:

And Cas has already been shot in the heart, metaphorically, because he, an angel, has fallen in love with… humanity (specifically, Dean). Which already brings him closer to earth and further from Heaven, because that kind of love is human.
