Those nifty elongated glyphs appear to be Bosančica.
Bosančica, an extinct Bosnian variation of Cyrillic, created and used in Bosnia in the early Medieval age.
I just learned that tag searches on Tumblr are diacritic sensitive. Searching "Bosancica" doesn't bring up any posts, but searching "Bosančica" brings up this one (and no others).
Oh, and the nitpicker in me wants to point out that "Medieval age" is redundant. "Medieval" is the adjectival form of "Medium Ævum," or "Middle Age."

I found the photo on the left in thread on Slavorum.com. The one on the right is on Wikipedia Commons. At first I thought they were the same plate under different lighting conditions, but, the more I look at the details, the more I think the one on the left is a reproduction.
